Job 1: Primary Care Coordinator (Full-time) x 3 positions
Location: Aljunied/Jurong
About the Role
I-CARE PCN supports GP clinics under the Healthier SG programme by delivering team-based care and ancillary services. Due to service expansion, we are looking for Primary Care Coordinators (PCCs) to support service delivery across clinics and mobile sites.
You will work closely with doctors, nurses, clinic staff, and the PCN HQ team to ensure smooth operations and a positive patient experience.
Key Responsibilities

Coordinate ancillary services such as:
-
Diabetic Retinal Photography (DRP)
-
Diabetic Foot Screening (DFS)
-
Nurse Counselling
-
Spirometry
-
-
Support patient registration, appointment scheduling, and service flow
-
Provide basic patient education and assist clinicians during service delivery
-
Handle patient enquiries in a professional and patient-centred manner
-
Perform documentation, data entry, and filing
-
Liaise with clinics and HQ on operational and administrative matters
-
Support audits, reporting, and general administrative tasks
Requirements
-
Diploma in Healthcare, Nursing, Life Sciences, Biomedical Science, or related field
-
1–3 years of healthcare or service-related experience preferred
-
DRP / DFS certification is an advantage (training will be provided if required)
-
Basic to intermediate Microsoft Office skills
-
Bilingual in English and Mandarin (dialects are an advantage)

Able to work well in a team and in a fast-paced environment

Job 2: Diabetes-Trained Senior Nurse Counsellor (Full-time / Part-time / Contract)
Location: Islandwide (GP clinics & mobile services)
About the Role
I-CARE PCN supports GP clinics under the Healthier SG programme by delivering structured chronic disease care in the community. We are looking for a Diabetes-Trained Senior Nurse Counsellor to lead and deliver nurse-led counselling, case management, and service development across clinics.
This role is suitable for an experienced nurse who is confident in chronic disease counselling, escalation, and clinical coordination.
Key Responsibilities

Provide structured nurse counselling using motivational interviewing, including education on diabetes and other chronic conditions

Conduct case management for complex chronic disease patients, including care planning and follow-up
-
Educate patients on self-monitoring skills (e.g. blood glucose, blood pressure) and track progress via follow-up calls

Perform clinical services where required, including:
-
Diabetic Foot Screening (DFS)
-
Diabetic Retinal Photography (DRP)
-
Spirometry
-
-
Identify and escalate priority or high-risk cases to doctors based on PCN protocols
-
Participate in developing structured programmes and care plans to improve chronic disease management
-
Contribute to PCN meetings, CME sessions, and case discussions with clinical input
-
Work closely with Primary Care Coordinators to support care coordination and transitions
-
Collaborate with management on quality improvement initiatives and service development
Additional Senior Responsibilities
-
Identify learning needs and oversee training plans for nurses and clinical PCCs
-
Develop and review competency and reassessment frameworks
-
Review and update nursing guidelines and escalation protocols in line with MOH guidance
-
Supervise and mentor nurses and clinical PCCs, providing guidance and feedback
-
Conduct routine reviews to ensure adherence to protocols and service standards
Requirements
-
Registered Nurse with relevant diabetes or chronic disease experience
-
At least 2–3 years of relevant experience in primary care, community care, or chronic disease management
-
Strong counselling, communication, and case management skills
-
Experience with DFS / DRP / Spirometry is an advantage
-
Comfortable working across multiple clinics and with multidisciplinary teams
